Insurance Plans We Accept for Eye Exams

Our practice accepts a variety of insurance providers for exams: Aetna, Ambetter/NH Healthy Families (Centene Vision), Anthem Blue Cross Blue Shield, Cigna, Davis Vision, Eyemed (Access/Insight/Aetna Networks), Harvard Pilgrim, Health Plans Inc., Humana, Medicare, New Hampshire Medicaid, Spectera, United Healthcare, US Family Health Plans at Martin's Point, VSP, and VSP Wellsense.

 **Please be advised that our empoloyees are not licensed insurance agents, which means that we are not able to advise any patient about which insurance they should sign up for during open enrollment periods.* *

Insurance Plans We Accept for Hardware (Eyeglasses and/or Contacts)

Our practice accepts five Eye Care Provider plans for "at point of sale" price reductions and discounts: Davis Vision, Eyemed Access/Insight/Aetna Networks, Spectera, VSP, and VSP Wellsense.

Hogan Eye Associates Financial Policy

The doctors and staff of Hogan Eye Associates, Inc. do not guarantee the accuracy of the benefit information provided to them by the patient's insurance company. It is the patient's responsibility to provide all insurance information prior to the appointment so their benefits may be verified, so that the exam and/or materials can be billed to their insurance company. Once billed to the insurance company, it cannot be changed or cancelled. The patient is responsible for any copays, coinsurance, deductibles, and balances that their insurance company does not pay, and all co-payments, balances and other applicable fees are due at the time of check-in. Any returned checks will result in a $25.00 fee. Hogan Eye Associates, Inc. will only bill insurances companies which they are contracted with. The patient is responsible for knowing who their insurance(s) is, and what their insurance(s) covers. The patient is responsible for knowing whether a referral from their primary care physician (PCP) is needed and for obtaining that referral. A 48-hour notice is required to cancel an appointment. Patients may be billed for appointments that are cancelled/missed with less than a 48-hour notice.
